Recognizing West Virginia's Best and Brightest Nurses

 

Purpose

To recognize our most competent, compassionate, and outstanding nurses in West Virginia

Expectations

Attendance at the Gala in Charleston, WV on September 24, 2022

Engage in at least one FONWV initiative (ex. serve on a board, mentor, team member, Ambassador, gala promotion...)

Nomination Process

• Begin by nominating a nurse on this form

• The nominator completes the Qualification Questions

• Nominee completes Nominee Questions

• Upload the  word documents and jpg photo to the Nomination Form

• Nominations are confidential: Names/identifiers are deleted for evaluators

• Nominations must be complete to be considered

• Previous  FON  Award winners may not be submitted for nomination.

• Nomination Deadline: FEBRUARY  14, 2022 at Midnight 

 

Selection Criteria:

• Currently licensed nurse in West Virginia

• At least 5 years of experience as a nurse demonstrating excellence in nursing practice & leadership

• Significantly or uniquely contributes to nursing through clinical practice, education, teaching, administrative
  governance, and/or healthcare innovation

• Demonstrates leadership/contributions in community affairs, nursing professional organizations, and/or mentoring
  other nurses; growth and development in these areas,

• Positively promotes and advances the nursing profession in the community (local/regional/national/global) through
  volunteerism, outreach or legislative/policy development,

• Nominee answer questions about nursing and expected future engagement in FONWV initiatives,

Narrative limited to a maximum of 500 words.

1. Describe how the nominee significantly or uniquely contributes to nursing through one or more of the following activities: clinical practice, education and teaching, administrative governance, and/or healthcare innovation (20 points).

2. Relate how the nominee demonstrates leadership or other contributions in one or more of the following areas: community affairs, nursing professional organizations or mentoring other nurses’ growth and development in these areas  (20 points).

3. Describe how the nominee positively promotes and advances the nursing profession at the local, regional, national, or global.

5. Describe, in your experience, the foremost quality that makes this nurse stand out as one of the most competent.
